본문/내용
또한 외상성 견관절 탈구의 초기 관절내 병변의 관절경적 관찰에 대한 보고도 현재까지 매우 드물다. Wheeler 등21)은 외상성 견관절 탈구 9례를 대상으로 한 첫 보고에서 전례에서 관절와 순의 전하방 박리와 상완골두 후방의 골연골성 결손 4례 및 연골성 결손 5례의 Hill-Sachs병변을 보고하였다. Norlin16)도 외상성 견관절 탈구 24례를 대상으로 수상후 3일 이내 관절경 관찰을 시행하여 모든 예에서 혈관절증 및 중,하 와상완 인대와 함께 박리된 관절와 순과 Hill-Sachs병변을 확인하고 그중 6례에서는 골연골성 결손, 18례에서 연골성 결손이라고 하였다. 이처럼 초기 보고된 외상성 견관절 탈구의 관절경적 관찰 보고에서는 Bankart병변이 모든 예에서 관찰되어 Bankart병변의 유무로 추후 재탈구를 예견할 수는 없으며 마취하 견관절 안정성 검사가 추후 재탈구와 밀접하다고 하였다. 이는 본 연구와 유사한 결과로 젊은 연령군의 전례에서 관절와 순의 박리가 관찰되었으나 손상 정도는 다양하였고 비교적 적은 범위의 관절와 순의 박리를 보인 1례에서는 마취하 견관절 안정성 검사상 안정성을 보였으며 관절경 수술시 관절와 순의 hook 견인에도 안정성을 보여 관…(생략)
