담도계질환의 병태생리 및 담석형성의 기전

1) 담관낭종(choledochal cyst)

담관낭종은 간외 또는 간내담도의 낭종성확장을 나타내는 선천적기형이다. Todani 분류법이 흔히 이용된다.
Type 1; fusiform dilatation
2; true congenital diverticulum
3; dilatation of the intraduodenal portion of the common bile duct(choledochocele)
4; multiple cyst
5; single or multiple dilatation of the intrahepatic duct(Caroli`s disease)
원인은 40%정도에서 anomalous junction of the pancreatic and common bile duct가 동반된다. 전형적 삼징후인 동통, 황달, 복부종괴촉지는 20%에서 나타난다. 진단은 ERCP가 가장 우수하며 전암병변으로 완전절제를 해야만 한다.
2. Dysmotility of the Sphincter of Oddi
오디괄약근 기능장애는 Oddi 괄약근에서 유발하는 양성의 비담석성 폐쇄성질환으로서 pancreatobiliary-type pain, cholestasis, 췌장염을 일으키기도 한다. 간혹 papillary stenosis, biliary dyskinesia, postcholecystectomy syndrome 으로 사용되기도 한다.
