다운증후군 아동은 다발성 기형 및 정신 지체를 동반하는데 이는 제21번 염색체의 별도의 물질의 존재에 의한다. 비록 임상적인 발현 양상은 다양하나 대개는 다운증후군의 진단을 의심해 볼만한 일치된 임상양상을 갖고있어 임상적으로 경험이 있는 의사는 쉽게 이 질환을 의심해 볼 수 있다.
가장 흔한 신체적 특징 중에서 저긴장증, 단두증, 내안각 췌피, 낮은 콧대, 작은 입모양, 작은 귀, 목 부위에 늘어진 피부, 손바닥에 하나의 가로지르는 손금 그리고 작은 다섯째 손가락이 짧으면서 휘어져 있는 형태 등이다. 또한, 첫 번째와 두 번째 발가락 사이에 깊고 넓은 공간이 존재하는 경우가 흔하다.
정신 지체의 정도는 다양하여 경도의 정신지체(지능지수 50-70)에서 중등도의 정신지체(지능지수 35-50), 그리고 때로 중증의 정신지체(지능지수 20-35) 등으로 다양하다. 또한, 선천성 심장질환(50%), 백혈병(1% 이하), 청력장애(75%), 장액성 중이염(50-70%), 선천성 거대 결장증(Hirschsprung disease)(1% 이하), 위장관 위축(12%), 백내장(15%) 과 심한 굴절성 이상(50%)과 같은 안과적 질환(60%), 그리고 후천성 고관절 탈구(6%), 갑상선 질환(15%) 등…(생략)
