골반경을 이용한 자궁근종절제술의 가장 큰 문제점은 meticulous suturing이 어렵다는 사실 이다. 그래서 intramural hematoma가 형성되어 흡수되면서 uteroperitoneal fistula가 생길수 있으며,(Nezhat and Nezhat, 1991) thermal energy의 광범위한 사용으로 인해 tissue necrosis가 야기되어 scar strength가 약해져 임신했을 때 uterine rupture가 생길 수 있다.
현재 laparoscopic myomectomy후 임신 3분기에 자궁파열이 발생한 3례의 증례보고가 있다.(Harris,1992; Dubinsson et al.,1995; Friedman et al.,1996) 이러한 자궁파열은 개복술하 여 자궁근종절제술후에도 보고되어 있다.(Rubin,1942; Georgakopoulous and Bersis, 1981; Golan et al.,1990) 그래서 Nezhat등은 laparoscopically assisted mini-laparotomy myomectomy를 함으로써 meticulous suturing이 가능하다고 제안하였다. 이러한 방법은 크기 가 크고 깊게 파묻혀 있는 intrmural myoma인 경우 multilayer suturing이 가능하므로 좋은 방법이나 broad ligament에 위치한 myoma, 자궁후벽의 myoma, 뚱뚱한 환자에서는 별도움이 되지 않는다.
